Manuscript Clearance Forms/Surveys

Required forms vary by degree type, so please choose the appropriate forms checklist for your degree type. All forms should be taken to the defense to obtain required signatures. All forms must be submitted to the Manuscript Clearance Advisor's office in 314 Westcott. Forms must be received and approved by the clearance office no later than 5 p.m. on the date of the student's deadline (semester or 60-Day). Sending forms via campus mail is not reliable close to deadlines.
